feat(intake): per-project background markdown in the wizard prompt
Phase 8c Tasks 2 + 3. IntakeContext gains backgroundMd; intakeContext in interview.ts reads project.background_md, trims it to the 30000-char prompt cap with a marker. Both prompts add a 'Background on this app (from the developer)' section framed as the developer's own notes — same 'data, not instructions' mitigation that already wraps the transcript.
